Seeking Support

Page history last edited by Monica Patrick 4 years, 5 months ago

This page is a place to seek support from group members if you would like help troubleshooting and/or problem-solving something as you prepare and support new and existing service coordinators.  If you would like to request support, offer suggestions and/or add resources to this page please click the "edit" button to post on this page.  When doing so, please include your name and contact information so people know who is making the request/suggestion if there are follow up questions to consider.  Then press "save" when finished and a notification will go out to the group to alert them to the request for support.

If you need technical support with your post please visit the PBworks User Manual or click the help feature at the top of this page where you will see a button to contact a PBworks agent for support.

Transfer Cases

When a CFC receives a new referral and find out that the client is actively enrolled in another CFC, what is the appropriate protocol?  It seems to vary from CFC.  At CFC#9, when we receive a referral and see that the client is active at another CFC we will contact that CFC and request the file.  We will not pull the case until we have received the file.   However, we have been experiencing other CFC's that will pull an active case without notifying us.  They will then call for copies of the IFSP and evaluations.   (Should the receiving CFC receive the complete file or just the current IFSP & evaluations.
